2017 Tennessee Hemophilia & Bleeding Disorders Foundation Annual Meeting Registration Form

• Individuals with a bleeding disorder and/or families with a person in their household who has a bleeding disorder will be provided accommodations for two nights at the hotel with the cost of registration (registration to the Annual Meeting is limited to immediate family members of the person with the bleeding disorder living in the same household). THBDF will secure lodging for registered participants; you will receive your reservation confirmation from THBDF two weeks in advance of the start of the meeting.

• One room will be provided for each family. Per hotel rules, single registration can only be made for individuals who are 18 years of age or older and who have a credit card.

• Each family will be requested to provide a credit card to the Hotel at the time of hotel check-in for any incidentals and/or damage. THBDF is not responsible for damage, accidental or intentional, to hotel property.

• If you require special accommodations (i.e. handicap accessible room, rollaway bed, crib, etc.), please make note on this form below. The Foundation will make every effort to make these arrangements on your behalf. Any additional cost of special accommodations will be your responsibility.

The registration fee is due on or before Friday, May 5, 2017, for your registration to be complete and your hotel room to be booked. Registration forms received after May 5 may be placed on a waiting list. Please do not let the cost of registration prevent you from attending the Annual Meeting. Families who received a waiver for the 2016 Annual Meeting and did not attend the meeting (no show) will not qualify for a waiver in 2017.
